This Ontario population-based study (2012–2021) of over 1 million births found that severe maternal morbidity (SMM) occurred in about 27 per 1,000 births (nearly 3%). While most cases happened during labor (55%), a substantial portion occurred before (16%) and after delivery (29%). Severe hemorrhage was the most common complication overall, with sepsis prominent postpartum. Risk factors differed by timing—for example, younger mothers (15–19) had higher antepartum risk, while intrapartum and postpartum risks followed a U-shaped pattern by age. Overall, about 40% of SMM cases occurred outside the delivery period, highlighting the importance of monitoring across the full pregnancy and postpartum timeline.
